TEMPLE SINAI OF ROSLYN COMMITMENT TO HEALTH POLICY

In an effort to reduce the risk of COVID-19 transmission, individuals coming to Temple Sinai of Roslyn must adhere to the policy below. This measure, like face masks, hand washing and social distancing, is intended to protect the health of the community. According to the Centers for Disease Control and Prevention (CDC), COVID-19 has an incubation period of up to 14 days. This means that even if YOU feel fine, you could be unknowingly spreading the virus to the people around you for as long as two weeks. We appreciate your honesty.
I/WE WILL NOT come to Temple Sinai if:
  • I/we have had a fever of 100.4 or higher that day.
  • I/we have had close contact with or cared for someone diagnosed with COVID-19 within the last 14 days.
  • I/we have experienced any cold or flu-like symptoms in the last 14 days (fever, cough, shortness of breath or other respiratory problem).
If I/we meet the criteria to come to Temple Sinai, I/WE WILL:
  • Remain in the car for the duration of the service.
  • If exiting the car to use the restroom, I/we will wear a mask at all times. Mask must fit securely over nose & mouth.
  • Maintain a physical distance of at least 6ft from anyone who's not part of my household.

Please place your initials here to confirm acceptance of this TSR Commitment to Health Policy. I/we understand that the registration for Simchat Torah will not be accepted without this initialed form.
